A Case of Pure Red Cell Aplasia Terminating in Acute Lymphoblastic Leukemia

Abstract

|
A 1 year and 10 month old boy was diagnosed as pure red cell aplasia because of
severe anemia and marked bone marrow erythroid hypoplasia. The severe anemia
resolved partially without therapy, showing marked erythroid hyperplasia in two weeks.
But thereafter he became to be anemic again and needed multiple transfusions in spite
of steroid therapy In six months from his first admission he readmitted due to
generalized petechiae, nasal bleeding and fever. The bone marrow study revealed acute
lymphoblastic leukemia: pancytopenta and immature lymphocytes in the peripheral blood
and lymphoblasts more than 60% in the bone marrow. The lymphoblasts had small size,
clumped chromatin pattern and scanty cytoplasm. During the induction chemotherapy he
died of pneumonia.
The transformation from pure red cell aplasia to acute lymphoblastic leukemia is very
rare and probably this case is the first case in Korea. So we reported this case with
the brief review of the literature in the view point of the pathogenesis and
transformation from pure red cell aplasia to acute lymphotlastic leukemia.
